It is imperative that you complete a Pre-Anaesthetic Questionnaire as soon as you have confirmed the date of your surgery. This gives us important information about your general health and helps ensure your operation can go ahead safely. Failure to complete the questionnaire may lead to your operation being postponed on the day of surgery.

Why do we ask you to complete this?

Your anaesthetist and surgical team need to know about your overall health, medications, previous anaesthetics, and any relevant medical conditions in advance. This allows:

  • Identification and optimisation of any medical issues before surgery.
  • Planning of the most appropriate type of anaesthesia for you.
  • Reduction of the risk of day-of-surgery cancellations.
  • Smooth and safe journey from admission to theatre to recovery.

Pre-Operative Assessment Clinic

In a small number of cases it may be necessary for you to attend our Pre-Operative Assessment Clinic. This depends on:

  • The type or complexity of your surgery.
  • Your general medical health (for example, if you have pre-existing conditions such as heart disease, diabetes or lung disease).

At the clinic:

  • One of our Consultant Anaesthetists will meet you.
  • We will review your questionnaire and discuss any questions or concerns you have.
  • Investigations/tests may be arranged if needed to optimise your condition before surgery.
  • You do not have to be already booked for surgery to attend. If you are considering surgery but uncertain, you may book an appointment. (Please note: clinic attendance may involve a separate fee.)

What to do now

  • On receipt of your surgery booking confirmation, access the pre-anaesthetic questionnaire (online link or paper version).
  • Complete all sections honestly and thoroughly. Provide accurate details about your health history, medications, allergies, smoking/alcohol use, any previous anaesthetics.
  • If you are referred to the Pre-Operative Assessment Clinic, attend the appointment at the scheduled time.
  • Contact us immediately if you experience any new health issues (illness, injury, medication change) between completing the questionnaire and the day of surgery.

Useful Information

  • Ensure you bring a current list of all medications and dosages with you on the day of surgery.
  • Inform us if you have had recent illness (cold, flu, chest infection), as this may affect your anaesthetic risk.
  • If you smoke, quitting even a short time before surgery can help your recovery.
  • Be sure to inform us of any changes in your health after submission of your questionnaire.
  • If you suffer from sleep apnoea, bleeding disorders, or take blood-thinning medications, please emphasize this on your questionnaire.
